98954984a4 Avoid link-time chrono dependencies
timer used to be independent from chrono. The tiny dependency used in the implementation can be isolated defining BOOST_CHRONO_HEADER_ONLY

The reverted commit had the effect of backing out several previously committed fixes. Possibly operator error?

e3b6783e6c Eliminate default_format from header. This eliminates initialization issues at the cost of some additional overloads. Docs to follow. Suggested by Rob. This change was made simpler by combining and reorganizing so there are now only two source files.
