77854c8aac Optimize out the extra PUSH_EMPTY/APPEND when evaluating a list. Reduces the number of instructions executed by about 25%. About a 3-5% improvement in overall runtime.

1ca8db235b Quickbook: Restore quickbook toolset. 
So that it can be used with older versions. Also removed `dist-lib` as
it doesn't seem to do anything, and quickbook no longer pretends to be
usable as a library.
