Christopher Kohlhoff 751cec5701 Make BOOST_ASIO_CONCURRENCY_HINT_SAFE consistent with io_context default constructor. 
Also fixed docs for BOOST_ASIO_CONCURRENCY_HINT_UNSAFE_IO to reflect
that registration locking is still enabled.
