16b1c5eadc Fix display of certain documentation pages. 
Still requires the page to have a body tag which means that some pages
that are technically valid HTML (neither the body tag nor the head tag
are required).

106726a80f When processing rss feeds, don't skip whitespace as it was causing required 
whitespace to be skipped, and don't html decode the values, as they've already
been decoded by the XML parser.

0b0c43e213 Limit the images that get themed. 
At the moment the code for theming boostbook pages is replacing any png in an 'images' folder, which is removing the images from some documentation. As a temporary fix, only do this for certain filenames. Refs: #2221.

db49ee6690 Some files look like directory names, but really aren't. This both prevents such files from being redirected to an index.html file. And displays them within the appropriate context. (fixes 1753)
