Correct testing bugs:

either changing assert(...) or BOOST_ASSERT(...) to BOOST_TEST
(in my code only)
or adding "return boost::report_errors();" where it was clearly
missing (and a pure bug, in anyone's code).

Removed instance() keyword declaration mechanism and added

BOOST_PARAMETER_KEYWORD macro. The former just wasn't portable
enough, and the latter is more succinct.
Added BOOST_PARAMETER_MATCH_ARGS to clean up Borland workarounds.
