c45bce11a7 fixed some misuses of BOOST_NO_STD_ITERATOR_TRAITS, changing it 
to BOOST_NO_ITERATOR_ADAPTORS, which is now always defined. Too
many problems:
1. internal compiler errors on VC++
2. warning from is_convertible under g++
