6f5b384b93 Merged from index_dev. 
not_xxx predicates generators removed.
Added rtree constructor, insert() and remove() taking Range.
Added default translator for boost::tuple<>.
Each R*tree test divided into 2 files.
Docs updated/modified/fixed.

6b456b1fc7 remove() don't return if there is no root, assertion check is done in raw_remove().
