Add documentation placeholders for mutex (and friends) lock(), try_lock(),
unlock() methods.

Add documentation placeholders for future (and shared_future)
get_exception_ptr() methods.

Remove documentation for obsolete future (and shared_future) swap(), operator
safe_bool() and operator!() methods.
