27a60c17b6 Implement a preprocessed rule for generating preprocessed source files. Refs #5390. This is not yet complete as it is only implemented for gcc and msvc.

4853417035 Fix boost build standalone documentation. 
Boost build defines its own jamroot, so it wasn't picking up the
`boost.defaults` flag from the main jamroot.

91be3a33a8 Merge documentation fixes. 
* Use `doc/src/*.css` instead of `doc/html/*.css`.
* Remove wiki and people directories.
* Some documentation fixes.
* Left out `minimal.css` changes and boostbook changes because of clashes.

85df1ff454 Update various libraries' documentation build. 
Mostly to use the images and css files under doc/src instead of
doc/html, usually be deleting the settings in order to use the defaults.
Also add 'boost.root' to some builds in order to fix links which rely on
it.
